If you are an award winner or someone supporting the award winner(s) and attending the banquet you MUST fill out the form below.
Award Winners DO NOT PAY for their Dinner Ticket to the banquet. However; you are still required to fill out a form if you are attending by yourself, so that we have a correct count of people attending.
If you would like to attend the Hall of Fame & Award Banquet as a supporter of an award winner you are required to fill out the form and pay prior to attending. The Cost will be $40 for Adults and $20 for Children under the age of 10.

IMPORTANT: All Award Banquet Dinner Tickets must be purchased by Thursday March 9th, so that we can confirm how many people will be attending the banquet to the Elks.

- Option 2. Half Page (4.5" (W) x 3.5" (H) or 432 (W) x 336 (H) Pixels) $50
- Option 3. Full Page (4.5" (W) x 7" (H) or 432 (W) x 672 (H) Pixels) $100
